This privacy policy (“Privacy Policy”) describes the policies and procedures of REHAB AND REVIVE PHYSICAL THERAPY, INC. d/b/a/ Rehab and Revive (“the company” or “we” or “us”) relating to the information we collect when you - an end consumer or patient who has registered and receive our services, including clinical and/or physical therapy services.

 

We value the privacy and security of your “personal information”, which as used herein - means any information that identifies, relates to, or could reasonably be linked with you. 

 

The purpose of this Privacy Policy is to provide you with a description of our personal information collection, use, and disclosure practices. If you provide us with your personal information, or we obtain personal information about you from other sources, we treat your personal information according to this Privacy Policy.

Your health or medical information, such as symptoms, conditions, health providers, medications, prior physical therapy, treatment plans, and other voluntarily provided health information may be collected. We use this information to provide our various services to you.

We do not sell your personal information to third parties. We may, however, share your personal information with service providers who assist us with our business functions -- such as payment processing.

BUSINESS TRANSFERS

In the event that another company acquires Rehab and Revive or its assets, we reserve the right to include personal information among the assets transferred to the acquiring company.

CHILDREN

Our website is not intended for or directed at children under the age of 13. In addition, we do not knowingly collect information from children under the age of 13.
